Testosterone Optimization

Rhett Women’s Center has four different options:

Treatments

  • Subcutaneous Pellet Implant Therapy – Pellets are placed under the skin (usually the “love handle” area) through a 5 mm incision. Benefits are typically noted in about three weeks and last four to six months. Advantages –
    • Superior results as testosterone are delivered steadily.
    • Enhanced compliance pellets are inserted three to four times a year.
    • No risk of transference of hormone to family members or pets
    • Hormone levels are monitored by lab work.
  • Self-Injected Therapy – Patients or family members can inject testosterone into the subcutaneous tissue (thigh, love handles, or belly) two to three times per week at home. Advantages –
    • No downtime
    • Superior results
    • Very little pain
    • No risk of transference of hormone to family members or pets
    • Hormone levels are monitored by lab work.
  • Bioidentical Hormone Cream Therapy – Testosterone is self-administered daily in the comfort of your own home. Advantages –
    • No procedure or needles are required.
  • Clomid and/or hCG – hCG is a placental hormone. We use it “off label” to maintain testicular size and sperm production when on testosterone therapy. It is given by injection twice per week. Clomid is an oral medication approved for use in females to treat infertility. In males, it is used “off label” to block estrogen receptors in the hypothalamus, causing the hypothalamus to release more GnRH (gonadotropin-releasing hormone), which causes the pituitary to release more LH and FSH. This causes the testicles to increase testosterone and sperm production. Not all patients respond to Clomid therapy. Clomid is taken three times per week by mouth.

How do I get started?

Step 1 – Initial consult

  • Medical consult
  • Family history
  • Current medications
  • Symptoms that bother you the most

Step 2 – Blood tests

  • Complete Blood Count (CBC)
  • Metabolic profile
  • Lipids
  • Prostate-Specific Antigen (PSA)
  • Total and Free Testosterone

Step 3 – Treatment

  • Review labs
  • Discuss treatment options
  • Initiate therapy

Step 4 – Follow up

  • Check lab work – after about three months is mandatory
  • Follow up after four months with face to face visit

What to expect

Most men will begin to notice symptom improvement after three to four weeks, with optimal results in five to six weeks. Changes in erectile dysfunction and ejaculation may require up to six months to experience improvement. Even with optimized testosterone, there can still be problems with erections.

To help optimize results, look at the following

  • Sleep 7 to 9 hours every night
  • Regular exercise – do something 5 to 7 days per week
  • Standard bowel function (1 – 3 times per day)
  • Clean eating
  • Detoxification – at least twice per year
  • Optimal thyroid function
